Installation Qualification Protocol Template
IQ template covering documentation, installation, utilities, calibration and safety checks.
Purpose
To provide a template installation qualification (IQ) protocol for temperature-controlled equipment, environmental monitoring systems and associated utilities, establishing documented evidence that the equipment has been delivered and installed in accordance with its design specification, the manufacturer's recommendations and the user's requirements, and that all documentation, utilities and safety features are in place before operational testing.

How to complete it
- 1.Execute each check with a witness where the site procedure requires; record actual results and attach evidence; record deviations on VAL-F09.
- 2.Summarise results, deviations and their resolution in the IQ summary section of VAL-F03.
- 3.QA reviews and approves the executed IQ; OQ (VAL-P06) may start only when the IQ is approved or when open deviations are assessed as not affecting OQ.
Before you use it
- Replace every square-bracket token, starting with [LABORATORY NAME], [ADDRESS] and [EFFECTIVE DATE]. The full token list is in GD-01 Read Me First.
- Have the content technically reviewed against how your laboratory actually works, then approved by the responsible manager before it becomes a controlled document.
- Update the Word fields after editing so the table of contents and page numbers are correct: select all, then press F9.
- Record the document in your own document register and set its review date.
