UKAS Initial Assessment Preparation Checklist

Everything to have ready before the pre-assessment and initial assessment visits.

Purpose

A preparation checklist for the accreditation body's pre-assessment and initial assessment visits (and, with minor adaptation, for surveillance and reassessment), covering the documentation, records, people, facilities and logistics that assessors expect to find. It complements GD-02 and CHK-01.

How to complete it

  1. 1.Start at least three months before the planned visit; assign an owner to each item; review progress weekly.
  2. 2.Where an item cannot be completed, record the reason and what will be said to the assessment team.
  3. 3.Prepare a document pack (electronic) indexed by clause using GD-03 so that any requested document can be produced within minutes.
  4. 4.Verify the current accreditation-body process, forms and fees with the body itself; this checklist is guidance, not the body's requirements.

Before you use it

  • Replace every square-bracket token, starting with [LABORATORY NAME], [ADDRESS] and [EFFECTIVE DATE]. The full token list is in GD-01 Read Me First.
  • Have the content technically reviewed against how your laboratory actually works, then approved by the responsible manager before it becomes a controlled document.
  • Update the Word fields after editing so the table of contents and page numbers are correct: select all, then press F9.
  • Record the document in your own document register and set its review date.
