EMS Alarm Configuration Record

Alarm limits, delays, escalation paths and verification per monitored point.

Purpose

The record of alarm configuration for every monitored point in an EMS or equipment controller - storage limits, warning and alarm limits, delays, hysteresis, notification channels, recipients and escalation - together with the verification by challenge test of each alarm at OQ, annually and after change, per VAL-P04 and VAL-P08.

How to complete it

  1. 1.One row per monitored point per parameter; record the justification for limits and delays (mapping study reference, monitoring uncertainty, product allowance).
  2. 2.Record each verification: method (physical challenge / simulation through the real notification path), observed delay, channels that received the notification, escalation observed, and pass / fail.
  3. 3.Changes are made only under change control; record the change reference and re-verification.

Before you use it

  • Replace every square-bracket token, starting with [LABORATORY NAME], [ADDRESS] and [EFFECTIVE DATE]. The full token list is in GD-01 Read Me First.
  • Have the content technically reviewed against how your laboratory actually works, then approved by the responsible manager before it becomes a controlled document.
  • Update the Word fields after editing so the table of contents and page numbers are correct: select all, then press F9.
  • Record the document in your own document register and set its review date.
